Alabama Medicaid

Ages 5 to 21 only for Alabama Medicaid coverage. Medicaid referral forms MUST be faxed from the Pediatrician's office to 1-866-385-0482 to begin services. 

Blue Cross Blue Shield of Alabama 

Ages 5 and older for BC/BS coverage.

Please complete the Insurance intake form and return it to our office via fax 1-866-385-0482.

We also offer a pay scale for individuals that currently don't have any type of insurance.
